New Patient Information

Dear Patient,

We invite you to visit our office for a diagnostic examination and consultation. Dr. Zemelman will examine your mouth and read your x-rays. Your consultation will be as long as required by your presenting problem: it may be very brief for a single tooth problem or quite lengthy for a full-mouth reconstruction.

Dr. Zemelman will share his diagnostic findings with you and discuss your treatment options and plans. We will help you make an informed decision that will best serve your needs.

• Please bring your previous dental x-rays if you have them.
• We work with most dental insurance plans – please be sure to bring your ID card.
• Patients under the age 18 must be accompanied by a parent or a guardian for the first visit.
